The Family Services Program (FSP) Senior Case Manager will work with children, young people and their families experiencing complex issues by implementing, monitoring and reviewing client outcomes against the agreed goals of each Family Action Plan. Client engagement with the FSP case manager is on a voluntary basis and clients will be determine intervention goals with the assistance of the FSP Senior Case Manager.
In addition to this the role involves responsibility for supervision of students on placement (in consultation with the Team Leader, Family Services) coaching and mentoring of other case management staff within the team as required and representing Cultura on relevant working groups and at practitioner focused forums within the sector.
This position will be contracted on a permanent part-time basis, working 22.5 – 30 hours per week, and will be based from the Northern Community Hub in Norlane.
